Autumn Equinox Celebration Participant Form

  • Please complete this form and submit it as soon as possible to be included in upcoming promotions. Deadline to submit form is Monday, September 16. Don't forget to email a copy of your insurance certificate to Laurel or let us know if you don't have one. Thank you!
  • We do our best to make this a green event and reduce waste. All attendees will have a reusable glass, plate and fork. Please only bring recyclable items to serve beverages, soups or desserts.

  • Please describe the salad, dessert, beer or wine you will provide for us to showcase at the event. Include contact information so we can arrange pick up on Friday, Sept. 20.

    Liability insurance requirements: Every booth holder must maintain no less than the following insurance during the event: Commercial General Liability: $1,000,000 combined single limit per occurrence for bodily injury, personal injury and property damage. $2,000,000 aggregate.
